Short-term disability insurance program

Short-term disability plan 
A short-term disability insurance plan provides income replacement for up to six months if you are out of work for a non-occupational accident or illness. You have two options for coverage, and although this is a voluntary benefit, you must elect within 30 calendar days of your eligibility-hire date, a qualified life event or during the annual open enrollment period.

Maximum covered salary Option A: $55,714.

Weekly tax-free benefit

Pays 70% of your actual salary or the maximum covered salary of your chosen option, whichever is less, up to the maximum of the following:

  • Option A – up to $750.
  • Option B – up to $1500.
  • Option C – up to $2000.
  • Pays 66⅔% of base pay, up to a maximum of $897.43.
  • Reduced by the amount of any paid leave benefit received from ASU Payroll while you are collecting short-term disability benefits.

$5,000 life plus $5,000 accidental death and dismemberment.

Coordination of Benefits

 
  • Benefits are coordinated with Compassionate Transfer of Leave.
  • You can receive both Unum short-term disability benefit and ASU paid leave benefit.

Benefit payment is reduced by 100% of any paid leave benefit after the elimination period. As a result, you may receive only the $67.31 minimum benefit under the MetLife plan.

Benefits start date
  • Begins on the first day if hospitalized for 24 hours or more, or outpatient surgery.
  • 31st day if not hospitalized.

Unum life and accidental death and dismemberment document

  • Begins on the first day if a disability is due to a non-work related injury.
  • 31st day if due to illness/childbirth. If coverage is elected after the initial new; hire/eligibility enrollment period and disability begins during first 12 months of coverage, benefits start on the 61st day if a disability is due to illness or pregnancy.

Pregnancy benefits
  • C-section pays up to eight weeks.
  • Normal birth pays up to six weeks.

  • C-section pays from day 31 up to day 56.
  • Normal birth pays from day 31 up to day 42.

  • Disability due to injury is 26 weeks.
  • Disability due to illness is 22 weeks if benefits start on day 31 or 18 weeks if benefits start on day 61.
Pre-existing condition exclusion

A pre-existing condition is not covered the first six months after initial election or after option level increase.

 

Exception: Pregnancy is not considered a pre-existing condition. Can elect while pregnant and collect benefits when the baby is born. However, pregnancy complications are considered a pre-existing condition.

Portability

Upon separation from employment, the life-ad&d coverage is portable or convertible to an individual policy, however, the STD portion of the plan is not.

Long-term disability insurance program

A group long-term disability insurance plan provides income replacement if you are out of work for a nonoccupational or occupational accident or illness for more than six months. Your LTD coverage is included with your mandatory retirement plan: ASRS, ORP or PSPRS.

Maximum benefit period Until age 65 or until you are able to return to work after your disability is determined by Broadspire. Determined by your age at time disability occurs.
Monthly benefit
  • Minimum monthly benefit of $50 as long as your monthly income from LTD and other income sources do not exceed 100% of your monthly compensation at the time disability commences.
  • Pays 66 ⅔% of base pay as of the date of disability, reduced by deductible income.
  • Maximum $10,000 before reduction by deductible income.
  • Minimum monthly benefit of $100 or 10% of your LTD benefit before reduction by deductible income, whichever is greater.
  • Pays 66 ⅔% of the first $14,999 of pre-disability earnings, subject to deductible income.
Benefits start date
  • Begins on day 181 of continuous disability.
  • Benefit is paid monthly as long as the participant continues to meet eligibility.
  • 50% of the claim payment is taxable.

 

ASRS LTD Plan Guide

  • Benefit is paid monthly.
  • Claim payments are fully taxable.

Pre-existing condition exclusion
  • Most recent membership date prior to July 1, 2008: three-month pre-existing condition limitation.
  • Most recent membership date on or after July 1, 2008: six-month pre-existing condition limitation.

Recurring disability benefits Periods of disability due to the same cause count as one period, unless separated by six months or more of full-time active work.

Periods of disability due to the same cause count as one period if:

  • Returned to active work for a period of 60 days or less, prior to completing the elimination period.
  • Returned to active work for a period of 180 days or less, after completing the elimination period.
Return to work benefits Partial benefits if working while disabled and receiving monthly benefits. Partial benefits if working while disabled and receiving monthly benefits.
