Updating beneficiaries

Naming a beneficiary requires thought and planning. Consult with an attorney for guidance, if applicable. Review and update your beneficiary designations on an annual basis, and more frequently if your personal circumstances change. 

You can designate the following to be your primary and secondary contingent beneficiary:

  • A charity.
  • One or more persons.
  • The trustee of an established trust.
  • Your estate.

A.R.S. §14-2804 revokes any provisions naming your ex-spouse as a beneficiary or as an executor, conservator or guardian. Unless your will states otherwise, your divorce automatically revokes any provisions naming your ex-spouse as a beneficiary or as an executor, trustee, conservator or guardian. You can designate your ex-spouse as your beneficiary after the date of your divorce, indicating a new relationship — i.e., friend.

How to add or make a change
Your life insurance plan enrollments must be processed before you can designate a beneficiary.
Follow the steps below to add or change your primary or secondary beneficiary designation online. If you designate a spouse, a child or children, you must be able to provide their social security number or taxpayer ID.

  1. Log in to My ASU using your ASURITE ID and password.
  2. Select the Faculty or Staff view located at the top right of the screen.
  3. Go to My Employment > Benefits > My Benefits Summary. In your benefits summary, you will see your elected life insurance. 
  4. Select the life insurance plan you wish to update.
  5. Select edit at the bottom of the page.
  6. Select Add a New Beneficiary. Follow the table for form field instructions. 
    New beneficiary Estate, charity or trust
    1. First name
    2. Last name
    3. Date of birth
    4. Gender
    5. Social security number
    6. Relationship to employee
    7. Select status for marital, student, disabled or smoker.
    8. Select box for  Same Address as Employee or Edit Address.
    9. Select the Same Phone as Employee or enter a phone number.
    1. First name field: Enter the last, then the first name listed on trust. 
    2. Last name field: Enter Living Trust or Estate
    3. Date of birth field: Use date trust or estate was established.
    4. Gender field: Set it to male. 
    5. Social security number field: Leave blank. 
    6. Relationship to employee: Select Estate from the dropdown menu. 
    7. Skip Status Information section.
    8. Select box for Same Address as Employee.
    9. Select the Same Phone as Employee or enter a phone number.
  7. Select Save, then OK. Verify your beneficiary or dependent information is correct.

How to change beneficiaries and allocations

  1. Follow the log in steps one through five above the table on the page. 
  2. Enter new allocations in the Allocation Details table, then click Update Totals. Totals must equal 100%.
  3. Select Save, then OK.
  4. Select My ASU to return to your dashboard.
  5. Repeat these steps for additional life insurance plans.

Prior dependent or beneficiary information will remain attached to your record and cannot be deleted. Name, date of birth and social security number for existing dependent or beneficiaries cannot be modified through My ASU once entered and saved. Submit a dependent or beneficiary revision form to make any updates.
