Payment Plan Fee Appeal

This form may be used to submit an appeal for ASU Payment Plan Fees ONLY. The applicable charges associated with the assessment of the ASU Payment Plan Fee are tuition, housing, food plans, parking permits, and health insurance. These fees must be paid prior to any appeal consideration.
