ASU ID Card mailing request form

This request is for ASU Online Students Only.

On campus students please use these directions for requesting an ID. 

Active online students enrolled for Summer or Fall 2024 may obtain an ASU ID Card by completing the following steps:

1. Submit your photo to Sun Devil Card Services.
2. Wait for your photo submission approval email.
3. Once approved, complete and submit this form.

You should receive your ASU ID card in the mail within 10 business days.

Cards will only be mailed to a U.S. mailing address.
