Basic life insurance and accidental death and dismemberment

The university provides benefits-eligible faculty and staff with a group term life insurance policy equal to one times your annual salary, rounded to the next $1,000. The first $15,000 of coverage is provided by the Arizona Department of Administration plan and the remainder is provided by the ASU plan. The maximum combined coverage amount across the two plans is $400,000.

Coverage amount  One times your annual base earnings, rounded up to the next $1,000, less the $15,000 ADOA plan. The minimum coverage is $5,000.
Coverage reduces beginning at age 70:
  • Age 70-74 reduction formula: annual salary times coverage level rounded to the nearest $1,000 times .40.
  • Age 75 and older reduction formula: annual salary times coverage level rounded to the nearest $1,000, times .25.
Reduced coverage is effective on the first day of the pay period in which the 70th or 75th birthday occurs.
$15,000 of coverage provided, regardless of salary.
