PTS license plate change or removal request form

Complete this form to request a change to a vehicle license plate in your ASU Transportation Account.

PTS may make the following changes to license plates registered in your account:

Incorrect license plates* will be amended in your account.
License plates not registered to your name* will be removed from your account.
License plates previously but no longer registered to your name, or those registered to your name but associated with another ASU Transportation Account, will be released of liability.
- These plates will remain non-active and visible to you.

* According to Department of Motor Vehicles records.
