Performance Improvement Plan

Performance improvement plans should be used when:
1. an employee has received a 2 or 1 on their performance evaluation (in a core expectation or overall); and/or 
2. coaching/counseling hasn’t resulted in acceptable performance.

NOTE: Units are encouraged to use these templates, but may also develop their own as long as ALL required elements are included.
